What you get

Entry at 7:30am via a side door. Small group (10-15 people). Guided walk through the main galleries with significantly fewer people. The Sistine Chapel between 8:15-8:45am has under 50 visitors, often under 30 in shoulder season.

What you don't get

You can still bring zero photos out of the Sistine Chapel (rule applies regardless of time). St Peter's Basilica is not always included; check before booking. Some early tours don't include the corridor exit to the Basilica.
